Optional Dependent Life and AD&D Insurance

Optional Life and Accidental Death & Dismemberment (AD&D) insurance is also available to protect your spouse (or state registered domestic partner) and/or dependent children. The amount of spouse Life Insurance is elected as a dollar amount of $5,000 or in $10,000 units to a maximum of $100,000. Your spouse must provide evidence of insurability for amounts above $20,000. Coverage may not exceed 50% of your own insured amount.

You may elect $5,000 or $10,000 of life insurance for dependent children. Coverage for a dependent child may continue through age 25 if unmarried. Coverage many not exceed 50% of your own insured amount.

Optional AD&D for Spouse Only (or state registered domestic partner) coverage is equal to 50% of optional AD&D coverage elected for yourself. Dependent Children Only coverage provides 15% of your own optional AD&D coverage per child. A Family option provides 40% of your own insurance as coverage for your spouse and 10% coverage per child.

Dependent Life and AD&D Insurance  
Spouse Life Insurance    $5,000 or in $10,000 units up to $100,000. Guaranteed acceptance amount is $20,000. Coverage over $20,000 subject to approval by Cigna.  Coverage is reduced at age 65 and again at age 70.
Spouse AD&D Insurance     50% of the employee’s optional AD&D election
Dependent Child Life Insurance  $5,000 or $10,000
Dependent Child AD&D Insurance  15% of the employee’s optional AD&D election
Family AD&D Insurance    40% of the employee’s optional AD&D election for a spouse and 10% per child
