The policy terms are renewed every year at the beginning of the Fall quarter/semester where price and coverage terms may change.
The 2013-2014 policy coverage is as follows:
Sickness visits and hospital care, lab, x-ray and testing services are covered according to the schedule of benefits in the brochure. There is in-patient and out-patient medical and surgical coverage. There is also prescription drug coverage. For details please see the 2013-2014 Insurance Brochure.
Routine dental treatment, eye exams (including glasses and contacts), hearing examinations, routine physical exams, acne, allergies, family planning, cosmetic surgery, preventative treatment and testing, elective treatments and surgery, injury caused by the use of alcohol and illegal drugs, and injuries resulting from participation in intercollegiate, club or professional sports.
Coverage is defined as "usual and customary" (URC) which means the average costs for medical expenses in the area/city you are seeking treatment.

Full-time international students are required to have the Seattle University Injury & Sickness student insurance plan OR complete a waiver form and provide documentation of their current valid health care plan to the International Student Center before the add/drop day of the quarter/semester.
If you do not complete the waiver by the deadline you will automatically be enrolled in the insurance plan and the fee will be billed to your student account.
Please be aware that coverage is not effective in your Home Country. For information about your travel benefits, please see the 2013-2014 insurance brochure.
You are allowed to visit any doctor of your choosing. Benefits limit doctor's visits to one visit per day.
Prescription drug coverage is included in the insurance plan, and you may go to any pharmacy. You must pay for the prescription and then file a claim form with Summit America Insurance Services. Be sure to include the pharmacy receipt that shows the name of the drug and the amount.
Complete a claim form and mail it to Summit America Insurance Services with a copy of the itemized bill showing the name of the drug and the cost.
You may pick up an ID card at the International Student Center (ISC). If you have any problems with your insurance card, please contact Summit America and let them know what the problem is and they will assist you in resolving it: by phone (toll-free) a 877.246.6997 or by email email@example.com. If you loose your card, please contact the ISC immediately for assistance at 206.296.6260.
